Prescription Request

Prescriptions and refills for established patients are routinely issued during our regular office hours, Monday through Friday between the hours of 8 AM and 5 PM.

Please make your prescription request by completing and submitting our Online Prescription Request Form below or by calling our office at (561)-499-0660 in Delray or (561) 395-6704 in Boca. You may also have your pharmacy fax the request to our office at (561) 499-4094 in Delray or (561) 362-9044 in Boca.

No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
